The Home & Community Support Innovation Centre is here to support North East Community Support Service (CSS) agencies in delivering the best quality care to clients through transformative system improvements and best practices. We are funded through Ontario Health North as a unique and innovative strategy to provide a model of excellence and the tools necessary for our Northeast Home and Community Care(HCC)/CSS sector partners to achieve success.

The First Link Intake Coordinator is responsible for connecting newly diagnosed people living with dementia and their families to the Alzheimer Society so that they can get the help they need as soon as possible.
The Product Management & Delivery (IAR, Assessment) Education Team at Ontario Health provides education and support for the use of interRAI standardized clinical assessments for new and experienced interRAI assessors working in the Community Support Services (CSS) sector to conduct quality assessments.
